What health insurance information do I need to schedule an appointment?


Please have and enter the following:
  1. Insurance company name
  2. Member ID number
  3. Group number(if applicable, Medicaid does not have a group number)
  4. Policy owner/subscriber (if the patient is not the primary person with the insurance, put the name of the person who is)
  5. Policy owner/subscriber date of birth (if the patient is not the policy owner, put the date of birth of the person who is)
